Wraparound Family Facilitator - [Duarte]

The Facilitator is the leader of the Wraparound Child and Family Team, which provides intensive services to families. The Facilitator convenes the Team to ensure the continuity of care and that all identified Wraparound services are provided.  The Facilitator serves as the contact point for assigned Wraparound families, service providers and the community, and maintains a caseload of up to 15 cases; services are provided in-home and in the community with 24 hour on call status for crises. The Facilitator will actively engage Wraparound families in the treatment planning process, work collaboratively with the rest of the Wraparound Team to develop and implement treatment plans, and monitor progress towards goals. Plans will be updated at least every six months with ongoing assessment.

Qualifications:

• Masters Degree in Social Work, Marriage and Family Therapy, or Psychology

• Registered or Licensed Clinical Social Worker, MFT, or Psychologist

• Bilingual – Spanish required 

• Excellent written and communication skills

• Experience in providing direct service and treatment to individuals, families and groups

• Provide excellence as a member of a team providing intensive/inclusive care

• Excellent interpersonal skills

• Ability to effectively and successfully interact with individuals from diverse socio-economic/ethnic groups

• Ability to keep information confidential

• Ability to be an excellent representative of the agency to the community
