WHAT IS Enhanced Care Management?

Enhanced Care Management (ECM) is a new Medi-Cal benefit that provides a care coordinator who will help with managing all of your physical and mental health services, to help you meet your health and wellness goals.

With Enhanced Care Management, you can expect:

  • Personalized care plans tailored to your unique needs

  • Coordination between healthcare providers, mental health specialists, and community resources

  • Assistance accessing medical, behavioral, and social services

  • Support navigating appointments, insurance, and follow-ups

  • Advocacy to ensure your voice is heard and needs are met

  • You’ll be supported through a program that’s part of California’s CalAIM initiative, designed to improve health outcomes across the state by providing coordinated, person-centered care.

  • Since ECM is a Medi-Cal managed care benefit, it’s available to eligible individuals at no cost to you, making it easier to access the support you need.

  • Most importantly, ECM helps by coordinating both your clinical care—like doctor visits and therapy—and non-clinical services such as housing assistance or transportation, so all parts of your health and life are cared for together.

Youth ages 0–21 and their families

Clients who are enrolled in one of Foothill Family’s six contracted Medi-Cal Managed Care Plans:

Molina Healthcare

Health Net

Before services begin, all clients will be screened by our ECM team to confirm eligibility and ensure the program is the right fit for your needs.

Program duration

Services are intended for 1 year. Duration can be extended based on family needs.
